It depends on whether your application is an initial application or renewal application.
For an initial application, you will need a completed DEQ disclosure statement, a copy of a color photograph of your self, a copy of your training certificate(s), and a copy of the two hour Arkansas Regulatory Awareness (if applicable).
For a renewal application, you will need a copy of your training certificate(s) and a copy of the two hour Arkansas Regulatory Awareness certificate (if applicable). A copy of the color photograph of yourself is optional for a renewal.
