NOTIFICATIONS, REPORTING FORMS AND INSTRUCTIONS

Forms for submitting Air Compliance Branch Notifications and Reports can be found under the Forms section of this page below.  To access forms, select (click on) the desired notification/report/form. Then:

  • To complete an form on-line, select the Begin Form Entry option at the bottom of the form.
  • To fill out and mail in a paper copy of a form, select the Download Mail-in Form option at the bottom of the form.

To submit some forms you must have e-signature approval.  To obtain this approval, you must download the DEQ electronic signature agreement (see additional links below) and mail it to DEQ.  E-signature does not convey any authorization or approval to submit or certify environmental records, such as a designation of Responsible Official.  E-signature is only an authentication of the person's identity.  Responsible Official requirements are contained in the on-line form and regulations.  Additional information can be found in our FAQ on e-signature approval.
